0402 MANUFACTURER OF LIQUEFIED PETROLEUM GAS APPLIANCES AND EQUIPMENT
  • Licensure is required for any person, firm or corporation involved in the manufacturing and/or sale of: tanks, cylinders or other containers and necessary appurtenances used to store, transport or deliver gas to the ultimate consumer; apparatus, appliances and equipment for use by consumers.
  • Application Fee: $525 / Annual Renewal Fee: $375
  • Other Requirements: Must provide proof of $1,000,000 (minimum) bodily injury and property damage liability insurance covering the products and operations of the business.
0404 CATEGORY III LIQUEFIED PETROLEUM GAS CYLINDER EXCHANGE OPERATOR
  • Licensure is required for any person, firm or corporation operating a cylinder exchange unit (where filled propane cylinders are stored while awaiting sale, or being offered in exchange for empty cylinders.)
  • Application Fee: $100 / Annual Renewal Fee: $65
  • Other Requirements: Must provide proof of $300,000 (minimum) bodily injury and property damage liability insurance covering the products and operations of the business; site plans must be submitted and approved; no examination required; final inspection of installed unit.
0405 LP GAS SPECIALTY INSTALLER E
  • Installation of portable propane cylinders of no more than 40 lbs. propane capacity at a consumer site for the sole purpose of fuel for outdoor appliances and equipment; servicing, altering or modifying outdoor appliances, equipment, piping, or tubing to convey LP gas to such outdoor appliances or equipment and selling or offering to sell, leasing or offering to lease, outdoor appliances and equipment for the use of LP gas.
  • Application Fee: $300 / Annual Renewal Fee: $200
  • Other Requirements: Must provide proof of $1,000,000 (minimum) bodily injury and property damage liability insurance covering the products and operations of the business; examination.
0407 INSTALLER B
  • Licensure required for any person, firm or corporation involved in the installation, service, repair, altering or modification of LP gas appliances and equipment attached to, or a part of a recreational vehicle.
  • Application Fee: $300 / Annual Renewal Fee: $200
  • Other Requirements: Must provide proof of $1,000,000 (minimum) bodily injury and property damage liability insurance covering the products and operations of the business; examination.
0408 INSTALLER C
  • Licensure required for any person, firm or corporation involved in the installation, service, repair and sales of LP appliances, equipment and piping. Does not include work performed on RV LP gas systems, LP gas containers or LP gas motor fuel systems.
  • Application Fee: $300 / Annual Renewal Fee: $200
  • Other Requirements: Must provide proof of $1,000,000 (minimum) bodily injury and property damage liability insurance covering the products and operations of the business; examination.
0409 REQUALIFIER OF CYLINDERS
  • Licensure required for any person, firm or corporation retesting, repairing, qualifying or re-qualifying U.S. DOT (ICC) specification tanks or cylinders.
  • Application Fee: $525 / Annual Renewal Fee: $375
  • Other Requirements: Must provide proof of $1,000,000 (minimum) bodily injury liability and property damage liability insurance covering the products and operations of the business; examination.
0411 FABRICATOR, REPAIRER AND TESTER OF VEHICLES AND CARGO TANKS
  • Licensure required for persons, firms or corporations involved in fabrication, repair, hydrostatic testing or re-qualifying of vehicles or permanently attached cargo tanks used to transport LP gases.
  • Application Fee: $525 / Annual Renewal Fee: $375
  • Other Requirements: Must provide proof of $1,000,000 (minimum) bodily injury and property damage liability insurance covering the products and operations of the business; examination.
0601 CATEGORY I LIQUEFIED PETROLEUM GAS DEALER
  • Licensure required for any person, firm or corporation involved in the following activities: sales of LP gas; cylinder exchange; sale or lease of LP gas appliances/equipment; installation, service and repair of LP or natural gas appliances and equipment; carburetion equipment sales and installation; re-qualification of cylinders.
  • Application Fee: $525 / Annual Renewal Fee: $425
  • Other Requirements: Must provide proof of $1,000,000 (minimum) bodily injury and property damage liability insurance covering the products and operations of the business; examination; minimum storage letter; submit site (with fee) plans for approval; registration of all vehicles used to transport LP gas.
0602 DEALER IN APPLIANCES AND EQUIPMENT FOR USE OF LIQUEFIED PETROLEUM GAS
  • Licensure is required for any person, firm or corporation selling or leasing liquefied petroleum gas apparatus, appliances or equipment.
  • Application Fee: $50 / Annual Renewal Fee: $45
  • No other requirements.
0603 INSTALLER D
  • Licensure is required for any person, firm or corporation involved in the conversion of vehicles to use liquefied petroleum gas as an alternate or primary fuel. Includes any installation, servicing, altering, modifying, or repairing of such equipment.
  • Application Fee: $300 / Annual Renewal Fee: $200
  • Other Requirements: Must provide proof of $1,000,000 (minimum) bodily injury and property damage liability insurance covering the products and operations of the business; examination.
0604 CATEGORY II LIQUEFIED PETROLEUM GAS DISPENSING UNIT
  • Licensure is required for any person, firm or corporation operating a liquefied petroleum gas dispensing unit for serving liquid product to the ultimate consumer; includes sales of appliances and equipment.
  • Application Fee: $525 / Annual Renewal Fee: $375
  • Other Requirements: Must provide proof of $1,000,000 (minimum) bodily injury and property damage liability insurance covering the products and operations of the business; site plans must be submitted (with fee) and approved; examination required; final inspection of installed unit.
0605 CATEGORY IV LP GAS DISPENSER AND RECREATIONAL VEHICLE SERVICER
  • Licensure is required for any person, firm or corporation operating a liquefied petroleum gas dispensing unit for serving liquid product to the ultimate consumer; includes sales of appliances and equipment, and whose services include service, installation or repair of recreational vehicle systems.
  • Application Fee: $525 / Annual Renewal Fee: $375
  • Other Requirements: Must provide proof of $1,000,000 (minimum) bodily injury and property damage liability insurance covering the products and operations of the business; site plans must be submitted (with fee) and approved; examination required; final inspection of installed unit.
0606 CATEGORY V LP GASES DEALER FOR INDUSTRIAL USES ONLY
  • Licensure is required for any person, firm or corporation filling, selling and transporting LP gas containers for use in welding, forklifts, or other industrial applications.
  • Application Fee: $300 / Annual Renewal Fee: $200
  • Other Requirements: Must provide proof of $1,000,000 (minimum) bodily injury and property damage liability insurance covering the products and operations of the business; site plans must be submitted (with fee) and approved; examination required; final inspection of installed unit.
0803 INSTALLER A
  • Licensure is required for any person, firm or corporation installing, servicing, modifying, altering or repairing apparatus, piping, tubing, appliances, equipment for use with LP or natural gas and LP gas storage tanks. This license includes the activities of selling or leasing of said equipment.
  • Application Fee: $300 / Annual Renewal Fee: $200
  • Other Requirements: Must provide proof of $1,000,000 (minimum) bodily injury and property damage liability insurance covering the products and operations of the business; examination.
0804 PIPELINE SYSTEM OPERATOR
  • Licensure is required for any person, firm or corporation who owns or operates a liquefied petroleum gas pipeline system that is used to transmit LP gas from a common source to the ultimate customer and that serves 10 or more customers.
  • Application and Renewal Fee: These fees are based on the number of systems owned or operated by the operator. A fee of $100 per system is assessed for up to four systems. No operator will pay more than $400 annually for licensure (See Section 527.02, F.S.).
