To obtain a Food Establishment Permit, you will need to complete a Food Establishment Permit Inspection Application and attach documentation of an approved water and wastewater system below. This must be done two weeks prior to the anticipated opening date. For public (municipal) systems, a copy of the water and sewerage bill or an application for water sewerage (for the location) will be required. Additional information will be required if you operate from an on-site well or wastewater septic system [ ].
At this inspection, our staff will ensure your business is eligible for permitting and in compliance with Chapter 500, Florida Statutes, and Rule Chapter 5K-4, Florida Administrative Code. Once your application is received, an inspector will contact you within three working days to schedule an appointment.
The Florida Department of Agriculture and Consumer Services is the exclusive regulatory and permitting authority for any person, business or corporation engaged in manufacturing, processing, packing, holding or preparing food, or selling food at wholesale or retail. Food is considered to include, but is not limited to, all prepackaged grocery items, prepared foods, packaged ice, bottled or vended water, vitamin and mineral dietary supplements, candy and other snack foods, soda and infant formula.
